The intervention purpose is to eliminate the facial tegumentary
excess, to re-lay the aponeurotic muscular system of the face
and to eliminate the wrinkles that can be found on the cheek
and on the neck.
When getting older, the ageing process manifests itself through
the shrinking of the facial bone structure and through the
changing of the way the fat deposits of the face are distributed,
the muscular tonicity changing and through the diminution
of the facial tegument elasticity. In this way a facial skin
excess appears leading to the formation of wrinkles, bags,
new creases and to the accentuation of the creases already
existing. Obviously it is necessary an elimination of the
tegumentary excess, a reinforcement, a repositioning and a
shortening of the fascial and muscular layer of the face.
Nowadays due to the development of the surgical techniques,
the actual technique has been radically changed, the intervention
addressing both to the tegumentary excess and to the aponeurotic
muscular and adipose facial layer, thus improving the results
and the lasting period of the facelift effects. At the skin
level there is the excision of the tegumentary excess through
the collection of the preauricular and retroauricular excess,
and at the facial aponeurotic muscular system level (SMAS)
there is the procedure of shortenings and skin foldings.

This intervention is addressed to subjects between 30-85 years
old, but the optimal age, when the there are maximum benefits
of the intervention, is between 35-55 years old. The best

About the results
By reducing the excess and by the suspension of the facial
aponeurotic muscular elements, the patient will gain a younger

be visible even after about 8-10 years.

The forehead. The frontal transversal wrinkles can be corrected
especially in the external part of the forehead. Also, with

The frontal vertical and horizontal wrinkles between the eyebrows
can not be corrected by the face lifting. This kind of wrinkles
necessitates an endoscopical intervention or an open intervention.
The cheek. The cheek can be corrected in the sense of lifting,
redistributing the fat and reducing the tegumentary excess.

Preoperatory consultation
The doctor will advise the patient on the results, on what
can be and can not be improved and on the scars that may remain.
The patient will report his/her clinical record, insisting
on the blood coagulation problems, allergies to drugs and
skin affections.
Analysis
The minimum analysis necessary before the operation: White
blood hemogram, Coagulation tests, Glycemia.
About the interventions
The intervention may take place under general or local anesthesia
with a IV sedation. In both cases there are no pains or other
displeasures. During the intervention under local anesthesia,
the patient is awake but is not stressed or anxious. It takes
about 1.5-5 hours.
For the global correction of the facial tegumentary excess,
the approach way (incisions) is done in the temporal, preauricular,
retroauricular regions and the pilose retroauricular region,
zones that permit the dissimulation of the scars. For the
minilifting the incision is made in the preauricular and retroauricular
zones.
During the intervention the suspension and the fasciomuscular
folding are performed on the same incision, using special
suture stitches, perfectly tolerated by the tissues. The tegumentary
suture is made according to the surgical technique details
that allow obtaining scars as less visible as possible, at
the anterior auricular level.
Hospitalization
The hospitalization lasts 24-72 hours and the postoperatory
care lasts for about 14 days, after which the subject can
resume her/his normal hygenical care.
The postoperatory period
The antibiotherapy, the anti-inflammatory treatment and the
cold application are present in the first 5-7 days.
The suture stitches are extracted by-stages, starting from
the 6th -7th postoperatory day and finishing in the 10th -12th
postoperatory day, when the bandage is completely removed
and the hygienic care can be resumed.
During the treatment, it is recommended the facial mimic and
the voice rest.
In the first 2-3 days moderate edemas emerge in the operated
zones, then starting from the 3rd day the process is followed
by resorption of edemas.
The ecchymoses (bruises) that appear in the first days can
last for about 14-15 days.
Postoperatory, the pains are inexistent or minimal and they
cede when using usual anti-inflammatories. The patient feels
a constant tension at the facial tegument level that cedes
in a few days.
About the complications
The complications are rare and reversible. They can be: haematomas,
small infections, large edemas and ecchymoses. The contusion,
the facial nerve injury are seldom found and are usually spontaneously
recuperated; if not, they beneficiate of specific treatment.
Recommendations
Sun exposure is recommended to be avoided in the first 3 months.
Postoperatory cosmetic procedures will be avoided for about
4 weeks.
The final result can be evaluated after about 3 postoperatory
months, when the resensitization of the tegument is complete
and the cervicofacial tissue is completely depenetrated. When
the hygenical cares are resumed, it is recommended to use
a mild shampoo, and the dying of the hair with less acrid
cosmetic products can be resumed after 8 weeks.
The social and professional activity can be resumed after
about 3 weeks.
