The
principle of the aesthetical rhinoplasty
The rhinoplasty is the surgical intervention that corrects the
shape and the size of the nose in relation to the individual
wishes and particularities of the features.
The rhinoplasty is the surgical intervention addressed to the
nasal pyramid and the airways, with corrective, reconstructive
and aesthetical purposes, and is mainly the duty of the ORL
doctor and of the specialized plastic surgeon.
Depending on the subject demands and on the local condition,
the rhinoplasty has a reductive, lifting effect. It corrects
asymmetries and deformities of the nasal lobe and of the body
of the nose.
The nasal pyramid is found on the center of the face, it is
tridymensional and it has many curved lines which also spread
in a tridymensional space. Models have been described satisfying
the aesthetic principles in rapport with the physiognomic marks.
The optimal length of the nose is a third of the total length
of the face. The nasal pyramid is symmetrical and median and
the nasal flaps do not exceed the inferior projection of the
internal borders.
The perfect nose only exists in relation to the individual features.

and a reinforced and tonic physiognomy. Postoperatory ecchymosis
can last for about 2-3 weeks and the nasal edema for about
2-3 months.
The final results can be seen after 3-6 months.
Adhesive tapes and splint are necessary for 6-14 days. This
intervention is addressed to subjects of both sexes that exceed
the age of 18 years old.
Consultation and analysis
The consultation will reveal to the doctor the biological
status of the patient as well as his/her motivations and hopes
regarding the result of the intervention. In the same time
the doctor will explain the elements of the intervention and
of the result with the purpose of clarifying to the patient
the options regarding the intervention and the results.
The minimum analysis necessary before the operation: White
blood hemogram, Coagulation tests, Glycemia, Frontal and Lateral
radiography of the nasal pyramid.
About the intervention
The intervention may take place under local or general anesthesia
and it requires minimum 12-72 hour hospitalization. The local
anesthesia allows you to leave cabinet sooner, but it has
the disadvantage that you are awake, and although you do not
feel pain, you feel the movements and vibrations during the
operation.
The intervention presumes the modification of the bone and
cartilaginous structure, by using an endonasal approach without
leaving visible scars at the exterior of the nasal pyramid.
About the complications
The complications are mild and controllable. The discontent
of the patient is the only unsatisfying result, which will
be corrected with a retouch after 10-12 postoperatory months.
Postoperatory
The anterior nasal tamponing for 72 hours; the nasal pyramid
edema is mostly reabsorbed in 14 days after operation and
completely reabsorbed after about 3 months. Periorbital ecchymoses
are reabsorbed starting with the 3rd -4th day and it lasts
up to 21 days.
Postoperatory, after the general anesthesia, the hospitalization
of the patient is necessary for about 12-72 hours.
The endonasal suture stitches are removed after 6-7 postoperatory
days. This procedure is painless.
The external adhesive tapes and splints are removed after
about 7-14 postoperatory days and then the hygienic care can
be resumed.
Antibiotic and anti-inflammatory treatment and cold applications
are recommended in the first 3 days.
Recommendations
In the first 3 postoperatory months, it is recommended to
avoid the exposure to the sun, to cold and to excessive heat.
The local traumas, even the minor ones, can influence, in
an unfavorable way, the shape and the modeling of the callus,
as well as the appearance of the tegument.
The nasal pyramid can be easily deformed. The bone and cartilaginous
callus are predominantly formed in the first three postoperatory
months and are modeled in about a year, when the final result
of the intervention can be also evaluated.
The social and professional obligations can be resumed in
about 2-3 weeks.
Postoperatory cosmetic procedures will be avoided for about
4 weeks.
About the results
The evaluation of the final results is made after approximately
3 months.
The immediate postoperatory aspect of the nose can be different
from the final one, depending on each individual capability
to construct the bone and cartilaginous callus.
